We ask first time patients to register with the receptionist. It is important that we obtain your entire medical record as well as insurance records at the first visit.
Occasionally a request is made to have your previous chart forwarded to our office. We require your permission to obtain these records.
If another physician referred you to our office, we automatically assume you will want us to send a report of the findings and recommendations. Please be assured that we keep your medical records strictly confidential and will not discuss them without your permission.

Please call for an appointment as early as possible to ensure the time and day that is most convenient for you.

Should you need to cancel or change the appointment please call the receptionist at least 24 hours in advance.

Unavoidable emergencies can occasionally delay your scheduled appointment. In the event of a delay, our receptionist will make every effort to contact you and reschedule your appointment if necessary.

Will there be students involved in my care?

Because of the affiliation with the University of Louisville School of Medicine, fellows, residents, and medical students in training may occasionally participate in your treatment. These knowledgeable professionals are an integral part of the health care delivery team and offer additional insights and observations to assist you.
